  4. Complete name(s), address(es) and nationality of the designer(s);
  5. Mode of the contract by which the design has been assigned to the applicant(s);
  6. First marketing date of the design;
  7. In case of priority claim:
    a) Priority date, country and serial no.
    b) Certified copy (copies) of the priority application(s) (term: within 3 months after the filing date;
  8. Power of attorney is to be filed.
